Liability Waiver & Oath of Sportsmanship

Release and Waiver from Liability Agreement

1. I acknowledge that I have volunteered to participate in the District Unity Games (“The Activity”). I understand that as a volunteer, I will not be paid or be compensated in whatever form or means for my services, that I will not be covered by any medical or other insurance coverage provided by the Church of Christ (Iglesia Ni Cristo), and that I will not be eligible for any Workers' Compensation benefits.

2. Release.

In consideration of my being allowed to attend or participate in the Activity, I hereby agree that I, my heirs, guardians, assignees, and legal representatives, will not hold the Church of Christ (Iglesia Ni Cristo), or either of their members, officers, employees, agents, and assigns, collectively or individually, or the supplier of any materials or equipment that is used in the Activity, for loss, damage or injury to myself or my property, resulting from or arising from my participation in the Activity.

I hereby waive and release any rights, actions, or causes of action resulting from loss, damage or injury to myself or my property, sustained in connection with my participation in the Activity, or in the travel to and from the Activity site. I assume full responsibility for myself and my property. I agree to indemnify and hold the Church harmless of any injury, loss or damage to myself or my property, due to the gross of ordinary negligence of the Church and its assigns.

I further consent to the unrestricted use by the Church of Christ (Iglesia Ni Cristo) and person(s) authorized by them of any photographs, recordings, interviews, videotapes, motion pictures, or similar visual recording of me.

If any portion of this Release shall be held invalid by law, the balance shall continue to be in full, legal force and effect.

I have read this release and fully agree to abide by its terms. (Parents should sign for minors)

Medical Clearance

I represent and warrant that I have undergone a physical examination by a licensed physician within [X days/months] prior to this event and have been medically cleared to participate in the Unity Games: Basketball in the District of Mountain States. I further represent that I am not aware of any medical condition, injury, or physical limitation that would make my participation unsafe, and I assume full responsibility for confirming my fitness to participate.

Oath of Sportsmanship

I hereby declare that I will completely follow all the rules and regulations of this sports activity.

I promise to adhere to the following:

  1. I will play competitively but will be ready to accept defeat.
  2. I will observe the biblical teachings of brotherly love and unity among members of the Church of Christ.
  3. I will maintain the highest level of Christian Sportsmanship toward my fellowmen.
  4. I will not allow myself to be a part of any kind of strife or discord throughout the entire activity.
  5. I understand that if I am found arguing, taunting, or in any way show offensive behavior, I will be asked to leave the event immediately and will no longer be allowed to participate in any of the Unity Games. Furthermore, I will also jeopardize and disqualify the entire team from participating in the remainder of the events.
  6. If I am found taking part in any kind of strife or discord, I will fully accept whatever disciplinary action the Church Administration deems fit to enact.
